Experiences and Reflections on My Visit to the Department of Peritoneal Cancer Surgery, 

Beijing Shijitan Hospital, Capital Medical University

The official cooperation agreement in the field of CRS and HIPEC between the Cancer Center of Beijing Shijitan Hospital, affiliated to the Capital Medical University, and The Norwegian Radium Hospital has been signed in April 2019 by Professor Yan Li and the representatives of The Norwegian Radium Hospital.

Following the detailed agreement regarding the exchange of surgeons between both hospitals in order to experience different surgical approaches to peritoneal metastases treatment, I was the first Norwegian visiting surgeon officially admitted at the Department of Peritoneal Cancer Surgery of Beijing Shijitan Hospital in the period 11.-24.10.2019. This visit was also important to further develop our designed international clinical trials regarding treatment of the peritoneal metastases patients with HIPEC and NIPEC.

My experience from this visit is exceptionally positive. I encountered a warm welcome from the Beijing Shijitan Hospital leaders as well as from Professor Li and his team. The schedule of the visit was well structured and precise. I took part in many advanced surgeries, observed the patients’ care directly after the surgeries and the follow-up period.

The surgery of peritoneal malignancies is an extensive and time-consuming procedure. A ten to twelve hours operation time is standard. The surgery contains omentectomy, total peritonectomy and resection of the organs affected by the tumor, frequently resulting in multivisceral resections. The goal of the surgery is to remove all visible tumor (cytoreductive surgery – CRS), and subsequently treating the patients with intraperitoneal chemotherapy (HIPEC, NIPEC).

Taking into consideration the severity of the disease, its advancement at the time of surgery and the usually poor prognosis, CRS and HIPEC/NIPEC are the only treatment methods that can considerably prolong the patient’s life, and, in selected patient groups, cure the disease.

At Beijing Shijitan Hospital operated with very skilled surgeons who performed advanced surgeries in a brilliant and sophisticated way, avoiding per-operative complications. I also witnessed their adherence to strict, personal follow-up of the patients after surgeries. The postoperative period is usually tough for the patients. I observed that the surgeons in charge monitored the patients immediately after surgery and provided them with firm instructions concerning physiotherapy and mobilizing.

I also saw very complex and elaborated training plans for residents, which are rigorously followed and controlled by the leader of the team - Professor Li.

I strongly hope that the collaboration between our hospitals will continue as we planned, and that we will establish several additional scientific projects in the future. “Learning from each other promote each other” is our goal. The unification of peritoneal malignancy surgery performed at Beijing Shijitan Hospital and at the Norwegian Radium Hospital is definitely possible, as there are more similarities in the performed procedures than differences. As I agreed with Professor Li, we will focus on frequent experience exchange, visiting each other, learning from each other and establishing clinical- and translational research projects in the future.

This process has already started during my visit in Beijing:

1. Another surgeon, Dr Milan Spasojevic, from the Norwegian Radium Hospital, is currently visiting Beijing Shijitan Hospital.

2. We are planning to start a randomized international clinical trial to be launched in the nearest future.

3. We are also organizing a long-term research position at the Norwegian Radium Hospital for Professor Li’s student.

As an old proverb saying goes: a good start is half done. This extraordinary first-hand experience at the Department of Peritoneal Cancer Surgery of Beijing Shijitan Hospital has already had positive impacts on our cooperation. And I firmly believe in the fruitful future of our long-term collaborations.

2019年4月,李雁教授代表首都医科大学附属北京世纪坛医院肿瘤中心腹膜肿瘤外科和挪威奥斯陆雷锭医院的代表正式签署合作协议,根据协议,中挪双方在肿瘤细胞减灭术(cytoreductive surgery, CRS)联合腹腔热灌注化疗(hyperthermic intraperitoneal chemotherapy, HIPEC)治疗腹膜癌领域展开合作与交流。

根据合作协议,双方互派外科医生到对方医院交流腹膜癌外科治疗经验。作为挪方派出的第一位外科医生,我于2019年10月11日至24日在首都医科大学附属北京世纪坛医院腹膜肿瘤外科正式交流、访问。这次交流和访问,进一步推动开展比较HIPEC 和常温腹腔灌注化疗(normothermic intraperitoneal chemotherapy, NIPEC)治疗腹膜癌疗效的国际临床试验。

我这次访问的感受是非常令人鼓舞。北京世纪坛医院的领导和李雁教授团队为我举行了热烈庄重的欢迎仪式。访问期间的日程安排紧凑、合理、精准。我参加了许多高难复杂的手术,直接观察术后和随访期病人的管理。

腹膜癌手术范围广泛,耗时长,通常需要10至12小时。这种手术包括网膜切除、全腹膜切除和受累器官切除,经常需要多脏器联合切除。手术的目的是切除所有肉眼可见肿瘤(肿瘤细胞减灭术),随后进行腹腔灌注化疗(HIPEC或NIPEC)。

腹膜癌病情严重且在手术时多为进展期,其预后较差,CRS 加HIPEC/NIPEC是首选的治疗方法,可明显延长生存,部分经筛选的患者可治愈。

在首都医科大学附属北京世纪坛医院腹膜肿瘤外科,外科医生技术娴熟,手术过程流畅、操作精准、配合默契,有效减少术后并发症的发生。我也目睹了他们严格管理术后患者。腹膜癌患者术后恢复通常非常艰难。我观察到,管床医生在术后立即给与患者运动指导,辅助患者进行心肺功能锻炼,促进心肺功能康复;指导患者进行下肢运动,预防静脉血栓的发生。我还看到了李雁教授团队有效、坚决实施住院医师培训计划,此培训计划复杂、详实。

我非常希望中挪两家医院的合作按计划实施,并在将来能多开展几项相关科学研究。“互相学习,互相促进”是我们合作交流的目标。在北京世纪坛医院和挪威奥斯陆雷锭医院开展标准统一的腹膜癌手术一定可以实现,因为我们之间有很多相似之处,而不同之处微乎其微。我非常同意李雁教授的观点,我们将重点进行经验交流、相互拜访、互相学习、开展临床和转化研究项目。

我在北京访问期间,以下项目已开始实施:

1. 另外一位来自挪威奥斯陆雷锭医院的外科医生,Dr Milan Spasojevic(米兰Ÿ斯帕索耶维奇),已开始正式访问首都医科大学附属北京世纪坛医院。

2. 我们计划在近期开展一项国际随机对照临床研究。

3. 我们为李雁教授团队的年轻医生在挪威奥斯陆雷锭医院组织确立了一个长期研究职位。

正如谚语所说:良好的开端就成功了一半。我在北京世纪坛医院腹膜肿瘤外科的难忘经历已经对我们的合作产生了积极影响,我也坚信我们的长期合作会在将来产生非常丰硕的成果。
